Default Claim forms

I have a claim that I have appealed and I wish to send additional evidence. Can I submit by just writing a letter or do I need to use a specific form?

The VA seldom requires a specific form. When they do and you didn't send one in, they will send you the form and ask you to fill it out. In this case, you can just write the RO a letter and tell them what you want. If you insist on a form, you can use VA Form 21-4138, Statement in Support of Claim. Your choice. The link provided leads to a fillable version of the form.

If you do elect to just send a letter, it is important to place your full name and SSAN on it so they can identify who you are.
